ABSTRACT

O objetivo desse estudo foi descrever a produção dos atendimentos oferecidos aos pacientes com necessidades especiais nos Centros de Especialidades Odontológicas (CEO) da Paraíba (Brasil) e sua relação com o cumprimento das metas de produtividade, entre o período de 2019 e 2022. Trata-se de um estudo descritivo e ecológico em que foram coletados dados secundários do Sistema de Informações Ambulatoriais do SUS (SIA/SUS), através da captação da produção ambulatorial individualizada (BPA-I), por meio da ferramenta TabWin, dos 98 CEO operantes na Paraíba. Realizou-se análise descritiva e analítica, por meio dos testes Qui-Quadrado de Pearson e Exato de Fisher entre a variável dependente "alcance da meta" e a variável independente "adesão à Rede de Cuidados à Pessoa com Deficiência (RCPD)". Em todos os anos, a porcentagem de CEO cumpridores da meta (15,3% em 2019; 1% em 2020; 12,2% em 2021; e 11,2% em 2022) foi substancialmente menor que os números expressados por aqueles que não alcançaram a produção mínima. Os resultados também apontaram maior realização de procedimentos restauradores (29,6% em 2019; 28,6% em 2020; 32,7% em 2021; e 37,8% em 2022) em detrimento aos periodontais, cirúrgicos e preventivos. No que concerne a estar aderido à RCPD, no ano de 2022, 90,9% dos CEO que alcançaram a meta estavam aderidos à Rede (p<0,05). Concluiu-se que uma baixa frequência de CEO alcançou o cumprimento da meta de produtividade da especialidade de Odontologia para Pacientes com Necessidades Especiais nos CEO. No entanto, a adesão à RCPD manifestou-se como elemento influenciador para aqueles que cumpriram suas metas mensais e anuais.


The objective of this study was to describe the production of care provided to special needs patients in the Dental Specialties Centers (CEO) of Paraíba (Brazil) and its relationship with the achievement of productivity goals, between the period 2019 and 2022. This is a descriptive and ecological study in which secondary data were collected from the SUS Outpatient Information System (SIA/SUS), by capturing the individualized outpatient production (BPA-I), through the TabWin tool, of the 98 operating CEOs in Paraíba. We carried out descriptive and analytical analysis, using Pearson's Chi-square and Fisher's Exact tests between the dependent variable "goal attainment" and the independent variable "adherence to the Care Network for People with Disabilities (RCPD)". In all years, the percentage of CEOs meeting the goal (15.3% in 2019; 1% in 2020; 12.2% in 2021; and 11.2% in 2022) was substantially lower than the numbers expressed by those who did not meet the minimum output. The results also indicated greater performance of restorative procedures (29.6% in 2019; 28.6% in 2020; 32.7% in 2021; and 37.8% in 2022) to the detriment of periodontal, surgical, and preventive procedures. Regarding being adhered to the RCPD, in the year 2022, 90.9% of the CEOs who reached the goal were adhered to the Network (p<0.05). It was concluded that a low frequency of CEOs achieved compliance with the productivity target of the specialty of Dentistry for Special Needs Patients in CEOs. However, adherence to the RCPD manifested itself as an influential element for those who met their monthly and annual goals.


El objetivo de este estudio fue describir la producción de la atención prestada a pacientes con necesidades especiales en los Centros de Especialidades Odontológicas (CEO) de Paraíba (Brasil) y su relación con el alcance de las metas de productividad, entre el período de 2019 y 2022. Se trata de un estudio descriptivo y ecológico en el que se recogieron datos secundarios del Sistema de Información Ambulatoria del SUS (SIA/SUS), mediante la captura de la producción ambulatoria individualizada (BPA-I), a través de la herramienta TabWin, de los 98 CEOs en funcionamiento en Paraíba. Se realizaron análisis descriptivos y analíticos, utilizando las pruebas Chi-cuadrado de Pearson y Exacta de Fisher entre la variable dependiente "cumplimiento de metas" y la variable independiente "adhesión a la Red de Atención a Personas con Discapacidad (RCPD)". En todos los años, el porcentaje de directores generales que cumplieron el objetivo (15,3% en 2019; 1% en 2020; 12,2% en 2021; y 11,2% en 2022) fue sustancialmente inferior a las cifras expresadas por los que no alcanzaron el rendimiento mínimo. Los resultados también indicaron una mayor realización de procedimientos restauradores (29,6% en 2019; 28,6% en 2020; 32,7% en 2021; y 37,8% en 2022) en detrimento de los procedimientos periodontales, quirúrgicos y preventivos. Respecto a estar adherido a la RCPD, en el año 2022, el 90,9% de los CEOs que alcanzaron el objetivo estaban adheridos a la Red (p<0,05). Se concluyó que una baja frecuencia de CEOs alcanzó el cumplimiento de la meta de productividad de la especialidad de Odontología para Pacientes con Necesidades Especiales en CEOs. Sin embargo, la adhesión al RCPD se manifestó como un elemento influyente para aquellos que cumplieron sus objetivos mensuales y anuales.

ABSTRACT Objective: To evaluate the oral health and related factors among people with visual impairment in a municipality in southeastern Brazil. Methods: A cross-sectional and descriptive household study was carried out on a representative sample of people with visual impairments in a medium-sized city in Brazil. A clinical-dental examination was performed (evaluating tooth loss, caries, periodontal health, dental plaque use and need of prosthesis) and a semistructured questionnaire was applied, investigating the socioeconomic conditions (sex, age, marital status, skin color and income) and the use of dental services (reason, time since, and type of service used [private/public] in last dental visit). Descriptive and bivariate analysis were performed to asses the factors related to oral health. Results: The mean age of participants was 65 years (±15.05), with the majority women (70.9%). 53.1% of the interviewees were edentulous, but 58.2% had a good perception of oral health. The blind ones use less public services (p=0.038) and with a lower frequency (p=0.014) than subjects with low vision. Those with less schooling had higher tooth loss and lower periodontal problems (p <0.05). There was a reduction of dental plaque six months after the educational action (30.7%; p=0.01), especially among the low vision participants (p=0.01). Conclusion: Severe tooth loss and unsatisfactory hygiene contrast with self-perception of oral health. Visual acuity and schooling were related to access and oral health. Actions for promotion and equity are necessary to overcome the inequalities encountered.


RESUMO Objetivo: Avaliar a saúde bucal e fatores relacionados entre deficientes visuais em município do sudeste brasileiro. Métodos: Foi realizado um estudo domiciliar, transversal e descritivo em uma amostra representativa de pessoas com deficiência visual em uma cidade de médio porte do Brasil. Realizou-se o exame clínico bucal (avaliando perda dentária, cárie, saúde periodontal, placa dentária, uso e necessidade de prótese) e aplicou-se um questionário semiestruturado, investigando as condições socioeconômicas (sexo, idade, estado civil, cor da pele e renda) e o uso de serviços odontológicos (motivo, tempo decorrido e tipo de serviço utilizado [privado/público] na última consulta odontológica). Foram realizadas análises descritivas e bivariadas para avaliar os fatores associados à saúde bucal. Resultados: A idade média dos participantes foi de 65 anos (±15,05), com maioria de mulheres (70,9%). 53,1% dos entrevistados eram edêntulos, porém, 58,2% apresentaram percepção de saúde bucal boa. Os cegos usam menos os serviços públicos (p=0,038) e em menor frequência (p=0,014) que os sujeitos com baixa visão. Os menos escolarizados apresentam maiores perdas dentárias e menores problemas periodontais (p<0,05). Houve redução de placa dentária seis meses após ação educativa (30,7%; p=0,01), sobretudo entre os sujeitos com baixa visão (p=0,01). Conclusão: Elevada perda dentária e higienização insatisfatória contrastaram com autopercepção dos agravos bucais. Acuidade visual e escolaridade relacionaram-se ao acesso e saúde bucal. Ações de promoção e equidade são necessários para superar desigualdades encontradas.

ABSTRACT

Objetivo: Pacientes com necessidades especiais são aqueles indivíduos que necessitam de cuidados especiais por tempo indeterminado ou por toda a vida. O presente estudo tem como objetivo abordar o atendimento odontológico de pacientes com necessidades especiais em uma instituição privada de ensino superior da cidade de São Paulo. Material e Métodos: Foi realizado um estudo descritivo, retrospectivo, para análise de 210 prontuários odontológicos de pacientes com necessidades especiais atendidos no Departamento de Odontologia para Pacientes Especiais da Universidade Cruzeiro do Sul entre 2012 e 2018. Dados sobre sexo, idade, diagnóstico da condição debilitante, motivo da consulta, uso continuado de medicamentos, imagens radiográficas e tratamentos odontológicos prestados foram todos coletados e posteriormente submetidos à análise estatística descritiva e inferencial (teste do qui-quadrado) ao nível de significância de 5%. Resultados: Quanto aos tipos de procedimentos odontológicos avaliados no presente estudo, dos 210 pacientes, 24% necessitaram de tratamentos curativos dentários em que a terapia periodontal foi o tipo mais prevalente, enquanto os tratamentos endodônticos tiveram a menor prevalência, representando apenas 4%. Houve alta prevalência de doenças crônicas sistêmicas, acometendo 56% dos casos, cuja faixa etária era superior a 40 anos. Conclusão: Check-ups odontológicos regulares devem ser priorizados em pacientes com necessidades especiais para evitar intervenções mais invasivas como em nosso grupo (AU)


Objective: Patients with special needs are those individuals who need special care for an undetermined period of time or for lifetime. The present study is aimed to address the dental care of special needs patients at a private higher education institute in the city of São Paulo. Material and Methods: A descriptive, retrospective study was performed for analysis of 210 dental records of special needs patients who were treated at the Dental Department for Special Patients of the Cruzeiro do Sul University between 2012 and 2018. Data on gender, age, diagnosis of the debilitating condition, reason of the consultation, continued use of medications, radiographic images and dental treatments provided were all collected before being submitted to descriptive and inferential statistical analysis (chi-square test) at a significance level of 5%. Results: As for the types of dental procedures assessed in the present study, of the 210 patients, 24% needed dental curative treatments in which periodontal therapy was the most prevalent type, whereas endodontic treatments had the lowest prevalence, representing 4% only. There was a high prevalence of chronic systemic diseases, affecting 56% of the dental records, whose age group was above 40 years old. Conclusion: Regular dental check-ups should be prioritized in patients with special needs to avoid more invasive interventions as found in our group (AU)

ABSTRACT

Abstract This study aimed to answer the following question: What is the proportion of acceptance reported by parents toward pediatric behavior guidance techniques (BGTs)? Observational studies that evaluated parental acceptance of BGTs during pediatric dental visits among parents of non-special health care need (non-SHCN) and SHCN children were included. A search of the Cochrane Library, Latin American and Caribbean Health Sciences (LILACS), MedLine/PubMed, PsycINFO, Scopus, and Web of Science databases, in addition to gray literature, was performed until October 2021. The Joanna Briggs Institute Critical Appraisal Checklist for Analytical Cross-Sectional Studies was used for quality assessment. The certainty of evidence was assessed using the Recommendation, Assessment, Development, and Evaluation (Grade). Fifty-three studies with 4868 participants were included, and 42 were retained for the random-effects proportion meta-analysis. The methodological quality varied from low to high. The agreement with the BGTs varied from 85.6% (95%CI: 77.5-92.1; p < 0.001; I 2 = 93.6%; 16 studies; n = 1399) for tell-show-do to 25.7% (95%CI: 17.8-34.4; p < 0.001; I 2 = 90.4%; 12 studies; n = 1129) for passive protective stabilization among non-SHCN children's parents; meanwhile, among the parents of SHCN children, it varied from 89.1% (95%CI: 56.1-99.7; p < 0.001; I 2 = 95.7%; 3 studies; n = 454) for tell-show-do to 29.1% (95%CI: 11.8-50.0; p = 0.001; I 2 = 84.8%; 3 studies; n = 263) for general anesthesia. The effect estimates varied greatly, as substantial heterogeneity across studies was observed, thus limiting the confidence in the results. Parents were more likely to agree with basic BGTs over advanced BGTs, with very low certainty of evidence. Dentists should discuss BGT options with parents. Objetivo:Fazer uma revisão integrativa a respeito do atendimento de pacientes com necessidades especiais (PNE) em Centros de Especialidades Odontológicas (CEO) no Brasil. Métodos: Para as buscas nas bases eletrônicas PubMed, BBO e LILACS, e Google Scholar, foram utilizados em português, espanhol e inglês, os descritores e termos livres: "pessoas com deficiência", "pessoa com necessidade especial", "pessoa com incapacidade", odontologia, "atenção secundária à saúde", "atenção secundária", "centro de especialidades odontológicas", CEO, "disabled persons", handicapped, "people with disabilities", dentistry, "secondary care", "dental specialty center" ,"secondary care centers", "personas con discapacidad", "atención secundaria de salud" e "centro de especialidad dental". Não foi feita restrição quanto ao idioma, mas o período consultado foi de 2016 a 2021. Incialmente foram excluídas as duplicatas, em seguida os textos cujos títulos e resumos não estivessem de acordo com os critéios de inclusão. Uma vez eleitos os textos a serem incluídos após leitura na íntegra, foi feita a extração dos dados de interesse: autor (ano), tipo de estudo, caracterização da amostra, local, objetivo, resultados principais e conclusão. A análise da qualidade metodológica e do risco de viés dos estudos foi feita por meio da ferramenta Quality Assessment Tool for Quantitative Studies do Effective Public Health Practice Project. Resultados: De 383 estudos, cinco foram incluídos. As barreiras mencionadas que dificultam o acesso dos PNE aos CEO se referiram a questões socioeconômicas e demográficas, localização dos CEO, escassez de recursos financeiros, limitações para acessibilidade e qualificaçao profissional deficitária para a prestação dos atendimentos. A qualidade metodológica foi considerada fraca em todos os estudos, o que indica alto risco de viés. Conclusão: Mesmo com a evolução gradativa do atendimento dos PNE nos CEO, ainda há aprimoramentos necessários, tanto em relação à qualificação dos profissionais para que tenham conhecimento e manejo para realizar os atendimentos seguindo os protocolos necessários, quanto à melhoria do acesso para esses pacientes.


Aim: To conduct an integrative review regarding the care provided to patients with special needs (PSN) in Dental Specialty Centers (DSCs) in Brazil. Methods: To perform searches in the PubMed, BBO and LILACS, and Google Scholar electronic databases, the following descriptors and free terms were used, in three languages of Portuguese, Spanish, and English: "pessoas com deficiência", "pessoa com necessidade especial", "pessoa com incapacidade", odontologia, "atenção secundária à saúde", "atenção secundária", "centro de especialidades odontológicas", CEO, "disabled persons", handicapped, "people with disabilities", dentistry, "secondary care", "dental specialty center" ,"secondary care centers", "personas con discapacidad", "atención secundaria de salud" e "centro de especialidad dental". No language restriction was made, but the period consulted was from 2016 to 2021. First, duplicates were excluded. After, texts whose titles and abstracts did not comply with the inclusion criteria were also excluded. Once the included texts had been chosen, and after reading them in full, the data of interest was extracted: author (year), type of study, sample characterization, location, objective, main results, and conclusion. The Quality Assessment Tool for Quantitative Studies of the Effective Public Health Practice Project was used to analyze the methodological quality and risk of bias found in the studies. Results: Of the 383 studies, five were included. The barriers that make it difficult for PSNs to access DSCs are related to socioeconomic and demographic issues, the location of DSCs, the scarcity of financial resources, limitations in accessibility, and deficient professional qualification to provide proper care. Methodological quality was considered weak in all studies (100%), with a high risk of bias. Conclusion: Even with the gradual evolution of the care provided to PSNs in DSCs, improvements are still necessary, both in relation to the qualification of professionals so that they have the knowledge and handling necessary to perform the care following the necessary protocols, as well as the improvement of access for these patients.

Abstract Objective: To evaluate the barriers to access and oral health care faced by children and adolescents with Cerebral Palsy (CP) according to their motor impairment through the perception of caregivers. Material and Methods: A case series study was carried out at three health institutions in Pernambuco, Brazil. The study sample consisted of 94 caregivers of 5-to-18-year-old patients with CP, according to GMFCS (The Gross Motor Function Classification System). Data were collected using a semi-structured form to evaluate the barriers to access and analyzed statistically by the chi-square and Fisher exact tests, adopting a 5% level of significance. In addition, binary logistic regression was performed to determine the weight of the variables in explaining the outcome variable. Results: There were major difficulties involving transportation (p=0.04) and structural accessibility to dental services (p<0.01) among children and adolescents with severe CP. In addition, the more severe the CP, the greater the difficulty of accessibility (OR=4.09,) and the lower the income (OR=8.80), the greater the motor impairment. Conclusion: Despite the availability of access to dental services, low-income families have more severe CP patients, contributing to the daily difficulties already faced by them in oral health care.

ABSTRACT

Patients affected with microcephaly have a head circumference below the average for sex and age, and in most cases it is associated with altered brain structure, developmental problems and dysmorphic facial features. Thus, the objective of this study was to describe the therapeutic approaches for dental care of patients with microcephaly. The selection of scientific articles for the study construction was carried out through the databases Scielo, Medline, Bireme and PubMed with a total selection of 16 scientific articles with years of cut off between 2010 to 2019. Microcephaly affects not only the patient's general health status, but also dental care, which is hampered by the small number of professionals qualified to care for special patients and the little knowledge of the craniofacial and dental characteristics of these individuals. In most cases, the dental surgeon is unprepared and insecure before these patients, which is hampered by the little knowledge they have of their main oral and craniofacial characteristics. Thus, the professional is unable to determine clinical procedures to be performed. Thus, it can be concluded that it is necessary for the dentist to have knowledge about microcephaly and provide instruction to the children's parents and guardians in this condition, thus facilitating the dental procedures execution. Os pacientes acometidos com microcefalia possuem um perímetro cefálico inferior à média para o sexo e idade, e na maioria dos casos está associado à estrutura cerebral alterada, problemas de desenvolvimento e características faciais dismórficas. Sendo assim, o objetivo deste trabalho foi descrever as condutas terapêuticas para atendimento odontológico de pacientes com microcefalia. A seleção dos artigos científicos para construção do trabalho foi realizada por meio de bases de dados Scielo, Medline, Bireme e PubMed, com seleção total de 16 artigos científicos com anos de corte entre 2010 a 2019. A microcefalia afeta não apenas o estado de saúde geral do paciente, mas também o atendimento odontológico, que se torna dificultado devido ao pequeno número de profissionais habilitados para o atendimento a pacientes especiais e ao pouco conhecimento das características craniofaciais e dentárias desses indivíduos. Na maioria dos casos, o cirurgião-dentista encontra-se despreparado e inseguro diante desses pacientes, o que é dificultado pelo pouco conhecimento acerca das principais características bucais e craniofaciais. Com isso, o profissional fica impossibilitado na determinação de procedimentos clínicos a serem realizados. Desta forma pode-se concluir que é necessário ao cirurgião-dentista um conhecimento sobre a microcefalia e proporcionar instrução aos pais e responsáveis pelas crianças nesta condição, facilitando assim a execução dos procedimentos odontológicos. ABSTRACT

El síndrome X frágil (SXF) es un trastorno ligado al cromosoma X, en el brazo largo Xq27.3, que provoca diversas alteraciones como problemas de conducta, deficiencia intelectual, macroorquidia, pabellones auditivos grandes y prominentes, paladar profundo y ojival, prognatismo mandibular, maloclusión y anomalías dentarias. El objetivo de este informe fue presentar el caso clínico de una paciente de 16 años con SXF, leucodermia, que se sometió a un tratamiento ortopédico funcional de los maxilares para la corrección del apiñamiento dentario. En el examen clínico se observaron timidez, ansiedad, inestabilidad emocional, trastornos conductuales esporádicos asociados a discapacidad intelectual leve, alteraciones craneofaciales y oclusales. Luego del estudio, evaluación radiográfica panorámica y trazados cefalométricos, se decidió instalar un dispositivo ortopédico funcional de maxilar, tipo Pistas Planas Indirectas, para posterior tratamiento ortodóncico correctivo. Bien al inicio del tratamiento se observó dificultad de comprensión y colaboración por parte de la paciente y su responsable (madre) y, luego de 5 meses, aún con mejoras en las funciones estomatognáticas, se inició el tratamiento ortodóntico con dispositivo fijo, el que fue concluido luego de dos años. El éxito del tratamiento de ortopedia funcional de los maxilares y/u ortodóntico, principalmente en el SXF, se basa en el abordaje comportamental y motivación en todas las etapas del tratamiento por el profesional, así como en un ambiente familiar colaborativo.


The Fragile-X Syndrome (FXS) is a disorder linked to X chromosome, on the long arm Xq27.3, causing several changes such as behavioral problems, intellectual disability, macroorchidism, large and prominent auricles, deep and ogival palate, mandibular prognathism, increased mandibular angle, malocclusion, and dental anomalies. The objective was to present a case of a 16-year-old patient with FXS, leukoderma, submitted to orthopedic functional maxillary treatment to correct dental crowding. In general, clinical examination, behavioral changes such as shyness, anxiety, emotional lability, sporadic disturbances of behavior associated with mild mental disabilities were remarkable. After panoramic radiographic evaluation and cephalometric tracings, it was decided to install the functional orthopedic appliance of the jaws, Indirect Flat Planes type, for later corrective orthodontic treatment. At beginning of treatment, there was a difficulty in understanding and collaborating, not only from the patient's side but also from the mother's. After five months, even with the improvement in stomatognathic functions, orthodontic treatment with a fixed appliance was started, which was concluded after two years. Success of functional and / or orthodontic jaw orthopedics treatment, especially in FXS, is based on behavioral approach and motivation in all stages, by the professional as well as a collaborative family environment.

ABSTRACT

Las personas con discapacidad son una población vulnerable y excluida de sus derechos como el ingreso, la permanencia y el egreso de la educación superior, especialmente de las facultades de Odontología en Latinoamérica. El objetivo de esta revisión es hacer una revisión de la literatura acerca de la inclusión educativa en las facultades de Odontología en América Latina. Se realizó unarevisión de la literatura nacional y latinoamericana al-rededor de los conceptos discapacidad, educación inclusiva y pregrados de Odontología, con fuentes primarias, secundarias y obras de referencia; entre las cuales se consultaron, libros, tesis y base de datos Medline, SciELO, Pubmed, Lilacs, Dialnet y Redalyc. Se concluyó que las facultades de Odontología están en mora y en deuda con las personas con (dis)capacidad en Latinoamérica por las dinámicas de la educación superior que siguen perpetuando la exclusión o la discriminación de personas con discapacidad en los pregrados de Odontología.


People with disabilities are a vulnerable and excluded population from their rights such as admission, permanence and graduation from higher education, especially from in Latin American dental schools. The objective of this review is to review the literature on educational inclusion in Latin American dental schools. A review of the national and Latin American literature was carried out on the concepts of disability, inclusive educa-tion and undergraduate dentistry using primary and secondary sources; among which, books, thesis and Medline, SciELO, Pubmed, Lilacs, Dialnet and Redalyc databases were consulted. It was concluded that dental schools are in default and in debt to people with (dis) ability in Latin America due to the dynamics of higher education that continue to perpetuate the exclusion or discrimination of people with disabilities in undergraduate dentistry.

ABSTRACT

En esta entrevista, el Dr. Roberto Elias nos revela cómo el trabajo odontológico con los pacientes especiales fue el principal motor para la fundación de AIOPE (Academia Interamericana de Odontología para Pacientes Especiales). Dicha asociación tiene como principal objetivo desarrollar actividades académicas que apoyan a las instituciones orientadas a la salud y áreas afines, así como plantear protocolos y políticas públicas para mejorar la calidad de vida de las personas con discapacidad. Asimismo, destaca la importancia de compartir las diferentes experiencias entre los especialistas de todo América y la importancia de incorporar asignaturas durante la enseñanza universitaria tanto en pregrado como posgrado para mejorar la salud bucal de las personas con discapacidad. Resalta el trabajo en equipo, así como conocer y escuchar a cada uno de los pacientes especiales, respetando su condición física, mental, sensorial y comportamental para humanizar la profesión odontológica especialmente dedicada a una población vulnerable.


In this interview, Dr. Roberto Elias reveals how dental work with special patients was the main driver for the foundation of AIOPE (Inter-American Academy of Dentistry for Special Patients). The main objective of this association is to develop academic activities that support health-oriented and related institutions, as well as to propose protocols and public policies to improve living conditions of people with disabilities. Likewise, it highlights the importance of sharing different experiences among specialists from all over America and the importance of incorporating courses during university education both in undergraduate and graduate degrees to improve the oral health of people with disabilities. In addition, teamwork stands out, as well as knowing and listening to every disabled patient, respecting their physical, mental, sensorial and behavioral condition in order to humanize the dental profession dedicated to a vulnerable population

Introdução: A hanseníase é uma doença tropical negligenciada causada por Mycobacterium leprae. O Brasil é um dos principais países endêmicos para a doença, persistindo como problema de saúde pública. Além disso, é a principal doença infecciosa causadora de incapacidades físicas. Objetivos: Analisar a magnitude e os fatores associados à limitação de atividade nos casos novos de hanseníase diagnosticados no centro de referência do nordeste do Brasil. Métodos: Trata-se de estudo transversal envolvendo 50 pessoas com diagnóstico de hanseníase. Foram coletadas variáveis clínicas e sociodemográficas além de aplicação da escala SALSA para análise da limitação funcional. Empregou-se regressão logística com cálculo de odds ratio. Resultados: A presença de limitações funcionais foi registrada em 32% (n=16) dos casos analisados, destacando-se sexo feminino (56,3%), idosos (37,5%), baixa escolaridade (87,6%), forma dimorfa (62,5%), classificação multibacilar (75,0%) e grau 2 de incapacidade física (50,0%). A limitação funcional esteve associada a faixa etária ≥45 anos (OR 3,80; p=0,047), classificação multibacilar (OR 4,28; p=0,021) e escore OMP ≥6 (OR 4,69; p=0,041). Conclusão: Observou-se elevada frequência de pessoas com incapacidade físicas. Os fatores associados à limitação funcional foram idade igual ou superior a 45 anos, classificação multibacilar e escore OMP maior ou igual a seis.


Introduction: Leprosy is a neglected tropical disease caused by Mycobacterium leprae. Brazil is one of the main endemic countries for the disease, persisting as a public health problem. In addition, it is the main infectious disease that causes physical disabilities. Objectives: To analyze the magnitude and factors associated with activity limitation in new cases of leprosy diagnosed in the reference center in Northeastern Brazil. Methods: This is a cross-sectional study involving 50 people diagnosed with leprosy. Clinical and sociodemographic variables were collected in addition to the application of the SALSA scale for the analysis of functional limitations. Logistic regression was used with the calculation of odds ratio. Results: The presence of functional limitations was registered in 32% (n=16) of the cases analyzed, especially female (56.3%), elderly (37.5%), low education (87.6%), dimorphic form (62.5%), multibacillary classification (75.0%), and degree 2 of physical disability (50.0%). Functional limitation was associated with an age range ≥45 years (OR 3.80; p=0.047), multibacillary classification (OR 4.28; p=0.021), and OMP score ≥6 (OR 4.69; p=0.041). Conclusion: There was a high frequency of people with physical disabilities. The factors associated with functional limitation were age equal to or greater than 45 years, multibacillary classification and OMP score greater than or equal to six.

Brasil es uno de los principales países endémicos de la enfermedad, persistiendo como problema de salud pública. Además, es la principal enfermedad infecciosa que causa discapacidades físicas. Objetivo: Analizar la magnitud y los factores asociados a la limitación de la actividad en los nuevos casos de lepra diagnosticados en el centro de referencia del noreste de Brasil. Métodos: Se trata de un estudio transversal en el que participaron 50 personas diagnosticadas con lepra. Se recogieron variables clínicas y sociodemográficas además de la aplicación de la escala SALSA para el análisis de limitaciones funcionales. Se utilizó regresión logística con el cálculo del odds ratio. Resultados: La presencia de limitaciones funcionales se registró en el 32% (n=16) de los casos analizados, especialmente mujeres (56,3%), ancianos (37,5%), baja escolaridad (87,6%), forma dismórfica (62,5%), clasificación multibacilar (75,0%) y grado 2 de discapacidad física (50,0%). La limitación funcional se asoció con un rango de edad ≥45 años (OR 3,80; p=0,047), clasificación multibacilar (OR 4,28; p=0,021) y puntuación OMP ≥6 (OR 4,69; p=0,041). Conclusión: Hubo una alta frecuencia de personas con discapacidad física. Los factores asociados a la limitación funcional fueron edad igual o superior a 45 años, clasificación multibacilar y puntuación OMP mayor o igual a seis.

Introduction General anesthesia is commonly used in the dental treatment of people with disabilities, anxious or difficult behavior, who need complex and/or extensive treatments. Objective The aim of this study was to evaluate the epidemiological profile of patients with disabilities undergoing dental treatment under general anesthesia by the team of the Dental Unit for Patients with Special Needs of the Federal University of Sergipe. Material and method Through the analysis of 371 dental records of patients assisted between August 2002 and March 2019. Result The analysis showed that 56.1% of patients were male, the most prevalent age group was 11-20 years, and the most frequent medical conditions were intellectual disability, followed by multiple disabilities. Relative to dental procedures, in 94.1% of the patients, it was observed that 4,056 dental extractions were performed and in 52.3% of patients, 1,156 restorations were performed. In 30.2% of patients, total multiple extractions were performed. Radiographic examination was performed in 51.8% of patients. As regards the origin of patients, 58.8% were from municipalities in the state, other than the capital. Return to the service for follow-up and prevention was observed in 38.5% of patients. Conclusion There is great need for early dental intervention, with specific preventive actions for this group of patients, focusing on those from interior regions of the state.


Introdução A anestesia geral é comumente utilizada no tratamento odontológico de pessoas com deficiência, ansiosos ou com difícil comportamento, que necessitem de tratamentos complexos e/ou extensos. Objetivo Avaliar o perfil epidemiológico da pessoa com deficiência submetida a tratamento odontológico sob anestesia geral pela equipe da Unidade de Odontologia para Pacientes com Necessidades Especiais da Universidade Federal de Sergipe. Material e método Através da análise de 371 prontuários de pacientes assistidos entre agosto de 2002 e março de 2019. Resultado Observou-se que 56,1% dos pacientes eram do sexo masculino, que a faixa etária mais prevalente foi de 11-20 anos de idade e que as condições médicas mais frequentes foram a deficiência intelectual, seguida pelas múltiplas deficiências. Quanto aos procedimentos odontológicos, observou-se que em 94,1% dos pacientes foram realizadas 4.056 extrações dentárias e em 52,3% dos pacientes 1.156 restaurações. Em 30,2% dos pacientes foram realizadas extrações múltiplas totais. O exame radiográfico foi realizado em 51,8% dos pacientes. Em relação a procedência dos pacientes, 58,8% eram provenientes de outros municípios do estado, distinto da capital. Os retornos ao serviço para acompanhamento e prevenção foi observado em 38,5% dos pacientes. Conclusão Há uma grande necessidade de intervenção precoce da odontologia, com ações preventivas específicas para esse grupo de pacientes, com enfoque no interior do estado de Sergipe.

Resumo Objetivou-se avaliar os cuidados em saúde bucal de crianças/adolescentes com Paralisia Cerebral (PC) segundo a gravidade, através da percepção dos pais/cuidadores. Estudo série de casos realizado em serviços de saúde de Pernambuco com 94 mães/cuidadores de sujeitos com PC entre 5 e 18 anos. Avaliaram-se fatores socioeconômicos, demográficos, cuidados em saúde bucal e uso de serviços odontológicos (SO). O Sistema de Classificação da Função Motora Grossa mostrou 67% com comprometimento motor grave. Indivíduos com PC grave tiveram frequência significantemente maior de pertencerem a famílias com renda mais baixa (89%, p < 0,001), residirem no interior (44%, p < 0,005), terem dificuldades de transporte (60%, p = 0,04) e acessibilidade aos SO (88%, p = 0,009) e maior necessidade de higiene bucal (67%, p = 0,008) realizada pelas mães (94%, p < 0,001). Apesar das barreiras de acesso aos SO, o mesmo foi facilitado para aqueles com PC grave, com uso precoce dos SO, mas com baixa oferta de dentistas e de humanização. Esses resultados informam problemas relacionados aos cuidados diários em saúde bucal, contexto de vida das famílias, de apoio institucional e de qualidade dos SO a serem enfrentados com políticas públicas socioeconômicas e de saúde integrais inclusivas e equânimes.


Abstract The aim was to evaluate the oral health care of children/adolescents with Cerebral Palsy (CP) according to severity through the perceptions of parents/caregivers. A case series study was conducted at health services in the state of Pernambuco, Brazil with 94 mothers/caregivers of subjects with CP from 5 and 18 years old. Sociodemographic factors, oral health care and use of dental services (DS) were evaluated. The Gross Motor Function Classification System showed 67% with severe motor impairment. Subjects with severe CP had significantly higher frequencies of belonging to families with lower income (89%, p < 0.001), living in the interior (44%, p < 0.005), having transportation difficulties (60%, p = 0.04), difficulty regarding access to DS (88%, p = 0.009) and a greater need for oral hygiene (67%, p = 0.008), which was performed exclusively by the caregiver (94%, p < 0.001). Despite identified access barriers, dental care was facilitated for those with severe CP, early DS use, but low availability of dentists and low degree of humanization were cited as major problems. These results reveal problems related to daily oral health care, family living context, institutional support and quality of DS that should be addressed in comprehensive, inclusive, equitable social and economic public policies.

ABSTRACT

Objetivo: identificar a percepção de cirurgiões-dentistas das redes pública e privada do município de Erechim, RS, sobre atenção em saúde a pacientes com necessidades especiais, avaliando conhecimento, preparo e limitações para o atendimento odontológico. Métodos: este estudo de caráter transversal ocorreu no período de março a agosto de 2019, por meio da aplicação de questionário próprio semiestruturado para cirurgiões-dentistas. Resultados: participaram da pesquisa 82 cirurgiões-dentistas, sendo a maioria (58,54%) atuantes na rede privada; 56,10% não cursaram uma disciplina específica em sua graduação sobre cuidados a pacientes com necessidades especiais. Em contrapartida, dos que cursaram, para a maioria, a disciplina era obrigatória e teórico-prática. Conclusão: apesar de somente 42,68% se sentirem muito bem ou bem preparados, somente 10,98% nunca realizaram atendimentos. Mesmo não tendo cursado uma disciplina específica em sua graduação, os cirurgiões-dentistas de Erechim, RS, realizam o atendimento e, quando necessário, o encaminhamento desses pacientes. Por manifestarem o interesse em se manterem atualizados sobre o tema, ações de educação continuada serão de fundamental importância.(AU)


Objective: assess dentists perception, about oral health care for patients with special needs, of public and private service of Erechim, RS, evaluating their knowledge, preparation and limitations for dental care. Methods: this cross-sectional research was conducted from March to August 2019, using questionnaires for dentists. Results: eighty-two dentists participated in the research, the majority (58.54%) are from the private service, 56.10% did not attend a specific discipline, in their graduation, about dental care for patients with special needs. In contrast, for those who attended, the discipline was compulsory and theoretical- -practical. Conclusion: although only 42.68% feel very well, or well prepared, only 10.98% never attended dental care. Even though they did not attend a specific discipline in their graduation, the dentists of Erechim, RS, perform dental care and, when necessary, they referral these patients. As they express their interest in keeping up to date on the topic, continuing education actions will be of fundamental importance.(AU)

RESUMEN Objetivo: Determinar las características clínico - epidemiológicas y necesidad de tratamiento de los pacientes especiales que recibieron atención en el Servicio de Estomatología de Pacientes Especiales de la Universidad Peruana Cayetano Heredia en el periodo de abril del 2016 a marzo de 2018. Material y Métodos: Se analizaron las variables procedencia, género, edad, diagnóstico sistémico y necesidad de tratamiento mediante estadística descriptiva. Resultados: La muestra estuvo conformada por 1 735 historias clínicas que cumplieron los criterios de inclusión. El 87,9% de pacientes viven en Lima Metropolitana y el 63 % proviene de Lima norte. El 62,2% fue de género femenino y el 37,8% de género masculino. La edad promedio de atención fue de 52,97 ± 25,92 años y el 50,26% fueron mayores de 60 años. Se obtuvo una media de 1,97 ± 1,21 diagnósticos sistémicos, con mayor prevalencia de las enfermedades que afectan al aparato circulatorio (35,3%) y las enfermedades endocrinas, metabólicas y nutricionales (26,4%). En cuanto a necesidad de tratamiento, destacaron la operatoria dental (75%), la rehabilitación oral (59,4%), y el tratamiento quirúrgico (36,7%). Conclusiones: La mayoría de la población especial atendida provino de la zona norte de Lima metropolitana, fue de género femenino y perteneció al grupo del adulto mayor. Además, las enfermedades más prevalentes fueron la hipertensión arterial (29,51%) y la diabetes mellitus (14,98%), necesitando principalmente tratamiento de operatoria dental y de rehabilitación oral.


SUMMARY Objective: Determine the clinical and epidemiological characteristics and need for dental care in the population of special patients that received attention in EPE - CEC - UPCH since April from 2016 to March 2018. Material and Methods: We analyze variables, such as the origin, gender, age, systemic diagnoses and need for treatment through descriptive statistics. Results: The sample were 1735 clinical files that met the inclusion criteria. The 87.9% live in Lima city and 63% in the northern zone, 62.2% were female and 37.8% male. The average age was 52.97 ± 25.92 years old, and 50.26% were older than 60 years. Special patient has 1.97 ± 1.21 systemic diagno-ses, with a higher prevalence of circulatory system diseases with 35.5% followed by endocrine, metabolic and nutritional diseases with 26.4%. Regarding the need for dental care, the main treatments were dental restorations (75%), oral rehabilitation (59.4%), and dental surgery (36.7%). Conclusions: Most part of the special population attended, came from the northern zone of Lima city, was female gender and belonged to the older adult group. In addition, the most prevalent diseases were arterial hypertension (29.51%) and diabetes mellitus (14.98%); and the main treatments were dental restorations and oral rehabilitation.

ABSTRACT

La pandemia por la COVID-19 ocasionó que los Estados promulgaran normas específicas para mitigar el riesgo de contagio y preservar la salud de la población; se establecieron e implementaron estrategias para disminuir la vulnerabilidad y cuidar los factores económicos y sociales. En este proceso, las personas con discapacidad (PcD) agudizaron su vulnerabilidad, pues desde que se decretaron las medidas para mitigar la condición de crisis social, se han identificado varios grupos con dificultades socioeconómicas. En general, muchas de las PcD se encuentran en estos grupos, pues su condición por sí misma los sitúa en circunstancias vulnerables, como efecto de las brechas de comunicación, acceso y oportunidad para los servicios de salud bucal. Así, el objetivo de este ensayo es analizar críticamente la situación de esta población en tiempos de pandemia. Se realizó una revisión de la literatura nacional e internacional alrededor de los conceptos discapacidad y COVID-19. Se concluye que la crisis agudizó la vulnerabilidad de las PcD generada por las escasas estrategias que se han implementado para cubrir esta población de acuerdo a sus necesidades, por lo que se incrementaron las barreras físicas y comunicativas y, con ello, limitado el goce efectivo del derecho a la atención de salud en los servicios odontológicos


The COVID-19 pandemic promulgated States to enact specific regulations to mitigate the risk of contagion and preserve the health of the population; strategies were established and implemented to reduce vulnerability and take care of economic and social factors. In this process, people with disabilities (PwD) increased their vulnerability, since the measures to mitigate the condition of social crisis were decreed; several groups with socioeconomic difficulties have been identified. In general, many of the PwD are found in these groups, since their condition by itself places them in vulnerable circumstances, as a result of gaps in communication, access and opportunity for oral health services. Thus, the objective of this essay is to critically analyze the situation of this population in times of pandemic. A review of the national and international literature around the concepts of disability and COVID-19 was carried out. It is concluded that the crisis exacerbated the vulnerability of PwD generated by the few strategies that have been implemented to cover this population according to their needs, thus increasing physical and communication barriers and, with it, limited the effective enjoyment of the right to health care in dental services

ABSTRACT

As doenças genéticas raras são consideradas eventos patológicos de origem genética de baixa ocorrência e com ampla diversidade de sinais e sintomas. Geralmente, indivíduos acometidos com doenças raras apresentam alterações musculares, esqueléticas e do sistema nervoso central. Muitas manifestações orofaciais são comuns nessa parcela da população, que, na grande maioria das vezes, apresenta dificuldade de acesso ao tratamento odontológico adequado. O conceito de integralidade do cuidado é amplamente discutido tanto nas práticas na área da saúde, quanto nas discussões relacionadas à compreensão do ser humano, de sua condição integral e não parcial. Este estudo objetivou identificar fatores associados ao acesso ao serviço de saúde bucal para indivíduos com doença genética rara e sem doença genética rara. Foi realizado um estudo transversal, pareado por sexo e idade, com 140 indivíduos [70 com doença genética rara - Mucopolissacaridoses (n=29) / Osteogênese Imperfeita (n=41) - e 70 sem doença genética rara] e os pais/responsáveis. A amostra foi selecionada em dois hospitais referência para pacientes com doenças raras em Minas Gerais, sudeste do Brasil. Os pais/responsáveis responderam um questionário sobre aspectos individuais (sexo, idade, cor da pele e escolaridade dos pais/responsáveis) e história médica e odontológica do filho (infecções respiratórias, uso rotineiro de medicamentos, acesso ao serviço de saúde bucal). O tipo de doença rara (MPS ou OI) foi confirmado pelo prontuário médico do paciente. O acesso ao serviço de saúde bucal foi analisado por meio da questão "Seu filho já foi ao dentista?" Os participantes com doença rara e sem doença rara foram examinados quanto à cárie dentária, má oclusão, anomalias dentárias e higiene bucal. Foi construído um modelo teórico por meio de Directed Acyclic Graph (DAG) para identificar possíveis variáveis de confusão na associação entre doenças raras e acesso ao serviço de saúde bucal. O estudo foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Minas Gerais. Foram realizadas análises de regressão logística condicional não-ajustada e ajustada (p<0,05). A faixa etária dos indivíduos examinados foi de três a 27 anos, com média de idade de 10,3 anos (±6,5). A chance de o indivíduo sem doença genética rara ter acesso ao serviço de saúde bucal foi 5,32 vezes maior (IC95%: 2,35-12,01). Indivíduos sem episódios de infecções das vias aéreas superiores (menos de seis meses) apresentaram 3,16 vezes maior chance de terem acesso ao serviço de saúde bucal (IC95%: 1,45-6,90). Concluiu-se que indivíduos sem doença rara e sem história de infecções das vias aéreas superiores (menos de seis meses) apresentaram maior chance de pertencerem ao grupo de indivíduos com acesso ao serviço de saúde bucal


Rare genetic diseases are considered pathological events of low-occurrence genetic origin and with wide diversity of signs and symptoms. Generally, individuals affected with rare diseases present muscle, skeletal and central nervous system alterations. Many orofacial manifestations are common in this part of the population, that in the vast majority of time it has difficulty accessing adequate dental treatment. The concept of comprehensive care is widely discussed both in health practices, then discussions related to the understanding of the human being, of his integral and non-partial condition. This study aimed to identify factors associated with access to oral health service for individuals with rare genetic disease and without rare genetic disease. A cross-sectional study was conducted, matched by sex and age, with 140 individuals [70 with rare genetic disease - Mucopolysaccharidosis (n=29) / Osteogenesis Imperfect (n=41) - and 70 without rare genetic disease] and parents/guardians. The sample was selected in two reference hospitals for patients with rare diseases in Minas Gerais, southeastern Brazil. Parents/guardians answered a questionnaire about individual aspects and medical and dental history of the child. Participants with rare disease and without rare disease were examined for dental caries, malocclusion, dental anomalies and oral hygiene. The theoretical model of the Directed Acyclic Graphs (DAG) was used to identify possible confounding variables in the association between rare diseases and access to oral health service. The study was approved by the Research Ethics Committee of Federal University of Minas Gerais. Unadjusted and adjusted conditional logistic regression analyzes were performed (p<0.05). The age group of the individuals examined was three to 27 years, with a mean age of 10.3 years (±6.5). The chance of the individual without rare genetic disease belonging to the group with access to oral health service was 5.32 times higher (IC95%: 2.35-12.01). Individuals without episodes of upper airway infections (less than six months) were 3.16 times more likely to be in the group with access to oral health service (IC95%: 1.45-6.90). It was concluded that individuals without rare disease and without history of upper airway infections (< 6 months) were more likely to belong to the group of individuals with access to oral health services.

Abstract The purpose of the present study was to compare the perceptions of the parents/ caregivers of young people with and without Mucopolysaccharidosis (MPS) with regards to their oral health-related quality of life (OHRQoL). A cross-sectional study was conducted with 29 individuals with MPS and 29 normotypic individuals aged three to 21 years and their parents/caregivers. All parents/caregivers of young people with MPS in follow-up at two reference hospitals in the city of Belo Horizonte, southeastern Brazil, were invited to participate in the study. Individuals without MPS were recruited from the pediatric clinics of both hospitals. Parents/caregivers answered a structured questionnaire addressing the sociodemographic characteristics, behavioral habits and medical and dental history of the children as well as the Brazilian short-form version of the Parental-Caregiver (P-CPQ). The individuals with and without MPS were examined for malocclusion, dental caries and oral hygiene by an examiner who had undergone training and calibration exercises. Mean age of the subjects was 12.1 years (± 4.2). Comparing total P-CPQ scores and scores on the oral symptoms, functional limitations and wellbeing domains, the parents/caregivers individuos with MPS reported a statistically significant greater negative impact on OS domain than their counterparts. Regarding the clinical variables, malocclusion was also associated a greater negative impact on OHRQoL of young people with MPS when compared to those of young people without MPS. Our findings show the great negative impact caused by the malocclusion of young people with MPS.

ABSTRACT

Objective: To characterize the profile of patients with special needs assisted at a Dentistry School of a Brazilian University. Material and Methods: A retrospective and descriptive study was carried out through the analysis of 329 dental records from individuals with special needs assisted at the Dentistry for Special Needs Patients discipline between 2012 and 2017. Data on gender, age, race, monthly family income, schooling, medical diagnosis of the disabling condition, drug used and type of dental procedures performed were reviewed. Data were analyzed by descriptive and inferential statistics (Shapiro-Wilk and t-student tests) with 5% significance level. Results: There was higher prevalence of females (50.5%) aged 45-54 years (18.7%), low schooling (36.2%) and monthly income of 1 minimum wage (61.9%). Most special needs were chronic systemic diseases (67.2%) and endocrine-metabolic diseases (16.7%). Among patients investigated, 66.3% used anti-hypertensive (38.3%) and anti-inflammatory (20.6%) drugs. Of dental procedures performed, curative procedures were the most prevalent in the majority of patients (90.6%). Conclusion: The profile of most patients with special needs was characterized by females with chronic systemic diseases and mean age of 39.6 years, low schooling and monthly family income of up to 1 minimum wage, whose main reason for the visit was the search for curative dental treatment.

ABSTRACT

Ectodermal Dysplasia (ED) represents a group of hereditary conditions in which anatomical structures derived from the ectoderm show defects in its development. It is observed aplasia or hypoplasia of the tissues, among them the changes of dental relevance, as the anomalies regarding the number and morphology of the teeth. Due to the particularity of the cases, the dental surgeon must adapt the operative techniques and the choice of dental materials to each individual, according to the perceived need and condition. The objective of the present study is to report the clinical case of a 10 - year old male patient with ectodermal dysplasia who was attended in the discipline of Patients with Special Needs of Dentistry of the Federal University of Fluminense and its multidisciplinary aspects. A Displasia Ectodérmica (DE) representa um grupo de condições hereditárias em que estruturas anatômicas derivadas do ectoderma apresentam defeitos em seu desenvolvimento. Observa-se aplasia ou hipoplasia dos tecidos, dentre eles as alterações de relevância dentária, como as anomalias quanto ao número e morfologia dos dentes. Devido à particularidade dos casos, o cirurgião-dentista deve adaptar as técnicas operatórias e a escolha dos materiais dentários a cada indivíduo, de acordo com a necessidade e condição percebida. O objetivo do presente estudo é relatar o caso clínico de um paciente do sexo masculino, de 10 anos de idade, portador de displasia ectodérmica, atendido na disciplina de Portadores de Necessidades Especiais de Odontologia da Universidade Federal Fluminense e seus aspectos multidisciplinares (AU)
